What to do in the first fifteen minutes
Wash the wound under running water with soap for a full fifteen minutes. Not a rinse — fifteen minutes by a clock. This alone removes a large proportion of the virus and is the single most useful thing anyone does.
Then apply an antiseptic if you have one, and do not close the wound with anything. Stitching a bite traps what you are trying to wash out.
Then come in, or ring. Same day, whatever the hour and however small the injury looks.
If you can do it safely, note what the animal was and where it happened. Whether it was a stray or somebody’s pet, and whether anyone can keep an eye on it, occasionally changes what is advised — though never enough to justify delaying the first injection while somebody goes looking.
Which animals, and which contacts count
Dogs are the main source here, followed by cats. Monkeys around the viewpoints and temple sites are a genuine risk and are bolder than people expect. Bats count too.
- Any bite that breaks the skin
- Any scratch that draws blood or breaks the surface
- Licking of an open wound or of the eyes, nose or mouth
- Any contact with a bat, however trivial it appears
A friendly, healthy-looking animal proves nothing. Rabies is infectious for days before an animal appears unwell.
The course after an exposure
Post-exposure vaccination is highly effective and is not optional if the exposure was real. It is given as a series of injections over roughly a month.
Somebody never vaccinated before needs the full schedule. Somebody who had pre-exposure doses needs only two, with no immunoglobulin — which is precisely why the earlier course is worth having.
Deep or multiple bites, or bites to the face, hands or genitals, also need immunoglobulin infiltrated around the wound, and that is arranged through a hospital — see hospital transfer.
Why waiting is the dangerous choice
Rabies has an incubation period of weeks to months, so nothing happens for a long time and people convince themselves it was nothing.
Once symptoms begin it is untreatable and essentially always fatal. There is no second chance and no experimental option worth pinning hope on.
The vaccine works because it is given before the virus reaches the nervous system. Every day of delay narrows that window.
Pre-exposure vaccination, before anything happens
Two or three doses before travel or early in a long stay. It does not make a bite a non-event, but it changes what a bite requires.
Two follow-up injections instead of a full course. No immunoglobulin, which is the component that is genuinely hard to obtain in remote places and expensive everywhere.

The wound itself, separately
Animal bites infect readily, and cat bites in particular drive bacteria deep through punctures that look almost harmless.
They are cleaned thoroughly, irrigated, and usually left open. Antibiotics are given where the site or the depth warrants it.

Children, who are bitten most
Children approach animals, are at face height with dogs, and frequently do not report a scratch because they expect to be told off for it.
Any bite or scratch on a child gets treated as significant, and the same fifteen-minute wash applies before anything else.

Completing the course after you leave
The schedule runs past most holidays, and that is expected rather than a problem.
You are given a written record with the exact dates each remaining dose is due, so any clinic anywhere in the world can continue it.
Do not let the course lapse because you are home and feeling well. Feeling well is the normal state throughout the incubation period.
