Burn Treatment in Phuket
Almost every burn seen on this island comes from one of three things: a motorbike exhaust, boiling water in a kitchen, or a beach fire show that went wrong. Burn treatment in Phuket starts with twenty minutes of cool running water, and what you do in that first half hour decides how the next month goes.
The first twenty minutes, which matter most
Cool running water over the burn for twenty full minutes. Not ice, not a cold pack, not butter, not toothpaste, not aloe from a bottle. Water, running, twenty minutes, by a clock.
This limits how deep the damage goes, and it works for up to three hours after the injury, so it is worth doing even if you have already wasted an hour deciding what to do.
Remove rings, watches and anything tight before swelling starts. Take off clothing unless it is stuck — anything stuck stays where it is until somebody with kit removes it.
Judging how bad it is
Superficial burns are red, painful and dry, and heal in a few days without a mark. Partial thickness burns blister, weep and hurt a great deal, and take two to three weeks.
Full thickness burns look white, brown or leathery and are strangely painless, because the nerve endings have gone. Those always need a hospital.
Size is judged against your own palm, which is roughly one percent of your body. Anything larger than a palm, or on the face, hands, feet, genitals or across a joint, gets seen rather than managed at home.
The exhaust burn, which is its own category
A calf against a hot exhaust pipe is the most common burn a doctor sees here, and it is almost always deeper than the person thinks because the contact was brief and the pipe was extremely hot.

Passengers get these more than riders, and almost always on the inside of the right calf. Long trousers do not prevent it and a helmet is irrelevant to it.
Blisters, and what to do with them
Leave small ones alone. Intact blister skin is a sterile dressing that nature supplied for free, and it protects the healing surface underneath.
Large, tense blisters, or ones over a joint where they will burst anyway, are better deroofed under sterile conditions than left to tear open in a hotel bathroom.
That decision is made when the burn is looked at. Bursting them yourself with a hotel sewing kit is how an ordinary burn becomes an infected one.
Dressings and the humidity problem
Burns are dressed with non-adherent materials and, depending on depth, silver-containing products that reduce infection and are changed less often.
Tropical humidity slows healing and encourages bacteria, so dressings here need changing more frequently than the same burn would at home.

Pain, treated properly
Partial thickness burns are among the more painful injuries there are, and the pain peaks during dressing changes rather than between them.
Analgesia is given before the dressing comes off, not after. Timing that correctly is most of the difference between a tolerable week and a miserable one.
Nobody is expected to be stoical about this, and asking for more pain relief is a reasonable request rather than a complaint.
Infection, and what it looks like
Increasing pain after day two, spreading redness beyond the burn margin, pus, a smell, a fever, or the burn suddenly looking worse than it did.
Any of those needs looking at the same day. Burn infections escalate faster than wound infections and are harder to reverse once established.
Tetanus status is checked at the first visit and topped up where it has lapsed, which for most travellers it has.
Which burns go straight to a hospital
Anything full thickness, anything larger than about the palm of a hand, and anything circumferential around a limb where swelling could restrict circulation.
Burns to the airway — soot around the nose or mouth, a hoarse voice, singed nasal hair after a fire — are an emergency regardless of what the skin looks like.

Children and scalds
Hot drinks and pans of water account for most burns in small children, and a child’s skin is thinner, so the same temperature does more damage.
The same twenty minutes of cool running water applies, and it is harder to achieve with a screaming toddler. Do it anyway, in a shower if that is easier.

Sun, sea and the healing burn
No swimming, no sea, no pool. Warm water full of bacteria on a healing burn is exactly the wrong combination, and it is the instruction people ignore most.
New skin burns in minutes and pigments permanently. Keep it covered and out of the sun for months, not weeks, or the scar will stay darker than the skin around it.
