Medical Certificate in Phuket
Insurers, employers, universities, airlines, visa offices and dive schools all want the same thing and all want it phrased slightly differently. Medical certificates in Phuket are issued in English, signed, with a licence number, on the same visit — and getting the wording right the first time is the whole trick.
What certificates are actually needed for
The requests fall into a small number of predictable categories, and each wants different information on the page.

What is on every certificate
Your name as it appears on your passport, the date, the findings, the diagnosis where one can be made, the treatment given, and any recommendation such as rest days or restrictions.
The doctor’s name, signature, medical licence number and the clinic’s details and stamp.
All of it in English. Thai-only documentation is a common reason a claim is bounced back weeks later.
Check the spelling of your name against the passport before you leave with it. A certificate reading Steven where the passport reads Stephen is enough for an insurer to send the whole file back, and correcting it three weeks later from another continent is far harder than correcting it at the desk.
Backdating, which cannot happen
A certificate records what a doctor found on the day they examined you. It cannot describe a week you were ill before anyone saw you.
Where you were unwell earlier and have evidence — a pharmacy receipt, a hotel doctor’s note, photographs, messages — that can be recorded as history, clearly labelled as reported rather than observed.
Nobody should be issuing anything else, and an insurer who spots one will reject the whole claim rather than just that line.
Insurance wording, which is fussier than people expect
Policies routinely require a diagnosis rather than a symptom, the date of onset, confirmation that the condition is not pre-existing, and a specific statement about whether travel was advisable.
“Gastroenteritis, onset 14 March, unfit to travel 14–16 March” is a claim. “Patient felt unwell” is a piece of paper.

Fitness to fly, and its opposite
Airlines have real rules about flying after certain conditions and procedures, and cabin pressure is a genuine clinical consideration rather than a formality.
Clearance to fly is issued after an assessment, not on request. Equally, a statement that you are not fit to fly is issued when that is true, and it is the document a policy needs to cover a change of flights — see fit to fly certificates.
Sort it days ahead. A departure gate is the worst possible place to discover a problem.
Diving certificates
Dive schools across the island want medical clearance for anyone who ticks a box on the standard questionnaire — asthma, heart conditions, ear problems, epilepsy, diabetes, recent surgery.
The examination covers the specific systems that matter under pressure, and the outcome is either clearance, clearance with conditions, or a clear explanation of why not.

Sick notes for work and study
Straightforward, and the requirements vary. Some employers want dates only, some want a diagnosis, some have their own form.
Ask your employer what they need before the consultation. Reissuing a certificate because it lacked one line is avoidable.
Emailed copies are provided as well as printed ones, because the printed one is going to end up at the bottom of a rucksack.
Visa and work permit medicals
Thai visa and work permit medicals have a prescribed format and cover a specific list of conditions. Other countries have their own.
Bring the actual form the authority issued. A general fitness letter is refused routinely, and then you pay twice.
